At the time of the inspection the caregiver did not know the designated program director. The caregiver named a different person as being their program director that is assigned to different sites.

At the time of the inspection it was found that the previously established responsibilities for the program director are not being followed by the program director assigned to the site. The staff at the site stated a different person was their program director.

At the time of the visit the most recent inspection form was not posted. The currently posted inspection is from 2025. There has been at least 1 inspection in 2026.

At the time of the visit an epi pen in the first aid kit had expired in November of 2025.

At the time of the inspection 2 persons were present for more than 5 days of care without a background check at the operation.

At the time of records review 1 of the 3 staff records reviewed did not have the 2985 Affidavit form signed by the caregiver.

It was found through the investigation that during active play a child ran into a wall, fracturing one wrist and breaking the other, and was not provided basic first aid for the injuries.

Through the course of the investigation it was found that two caregivers present did not have orientation to the site that would have helped them better handle an incident of a child having a serious injury.

Based on information obtained through the investigation, it was found that caregivers at the site did not have access to the electronic records. The paper files at the site were not updated to reflect added persons to the authorized pick up list for a child.

It was found through the investigation that the group was displaced from their normal class area and did not have immediate access to the first aid kit when a serious injury occurred.

A file reviewed does not have the begin and end date of employment for caregiver.

A file reviewed did not have annual training records available for review.

The CPR/First Aid of caregiver is not current.

There was a change in site director and the operation did not notify licensing within 5 days after change.

The operation did not practice the lock down drill 4 times in the school year. This was corrected at inspection when the operation practiced a lockdown while licensing was present.

The caregivers were unable to maintain physical proximity to all children.

The Program Director did not ensure all staff fulfill their assignments and responsibilities.

Children were waiting for 20 to 25 minutes before going to play outside. As a results children were observed out of control.

Children were allowed to play on the monkey bars.

A caregiver was unable to keep the children under control. The caregiver said she wouldn't be able to keep the children under control even if she ask them to stop.

A child had an accident while in care, caregiver unable to provide details of the accident.

It was observed the children out of control, children were running around, getting on tables and jumping off a stage located in the care area. As a result, a child had an accident while in care.

The program has not practice a Sheltering Drill during the year.

The program has not practice a Fire Drill during the year.

The program has not practice a Lockdown drill during the year.

It was observed a caregiver yelling at the children in order to get their attention or keep them under control.

Caregivers were observed unable to control the children, the children were waking around, running and unable to get in line without a caregiver raise her voice or threating them to call the parents.
